Share and compile your thought leadership accomplishments throughout the year! Below is an easy-to-use form that you can fill out each month to capture your latest thought leadership accomplishments, such as new publications, presentations, awards, grands, and community connections. The information collected here will be shared by the President during the periodical all-college addresses and compiled at the end of the year for the annual scholarship report. Additionally, the Provost Office faculty support team will use the information to identify opportunities to better support your work. A copy of all your entries will also be sent to you at your request for your Faculty Performance Evaluation. Please answer the questions below based on the publications, presentations or other forms of sharing scholarship and expert practice that you have completed in the academic year. For each accomplishment, you'll give the citation or other identifying information, and then answer 4 questions about that accomplishment. You can add up to ten (10) accomplishments at one time.
For publications, please give a full citation that includes author(s), date, title, and all information on the source (as relevant, journal title, issue, page numbers, publisher, etc.) For presentations, please give a full citation that lists all presenter(s) presentation title, name of the conference or the organization, presentation date and location.
