Chosen Name

National Louis University is aware that many of its students routinely use a first name or full name other than their legal name. As part of being a welcoming and inclusive campus, NLU will implement changes to increase the use of chosen name. Therefore, the university will permit any student who wishes to choose to identify themselves within the university’s information systems with a chosen name in addition to their legal name. It is further understood that the student's chosen name should be used in university communications and reporting, except where the use of the legal name is necessitated by university business or legal requirement.

What is a chosen name?

A chosen name is a first name (i.e., given name) that may be chosen to be used instead of legal first name. Students may opt to go by a name that is different from their legal first name. This name will appear instead of legal name in D2L and on communications with the student. Some records, such as paychecks, an official transcript, or financial aid, that require the use of a legal name, will not change to chosen name. However, whenever possible, chosen name will be used.

Students may choose to add a chosen name for any reason by submitting the form below. No documentation is required. If a student needs to change their legal name, they must complete the Change of Personal Information Form, which requires formal documentation.

NLU provides students with the option to not disclose their legal sex, or to remove reference to this designation. You may contact the Office of the Registrar at records@nl.edu to request that your legal sex be marked as unavailable in our data system and/or to also list a Gender Designation with the university. Students also have the option to indicate a personal pronoun. You must send your email from your NLU student email account to submit this request.
